Readopt Notes

As promised, I completed our readopt paperwork in time for the picnic, and we filed everything at the court this past Monday. I expect that they would have contacted us by now if something was amiss with the paperwork, so here is a list for what we submitted. For filing readopt paperwork at children's court in Monterey Park submit the following filled-out forms (window 2100, I got all forms off the web). DISCLAIMER: no guarantees, this is simply what worked for us:

1. $20 filing fee (check)
2. ADOPT-200, original plus 4 copies
3. Foreign Adoption Documents (adoption decree, birth certificate, abandonment report)
4. Copy of Home study and one post-placement report (we used the 6 months post-placement report)
5. FAM020 - Family Law Case Cover Sheet Certificate of Grounds for Assignment to District
6. Memorandum for Setting for Hearing (original plus 1 copy plus self-addressed, stamped envelope)
7. VS-44, Court Report of Adoption (original plus 1 copy)
8. ADOPT-210, unsigned (original plus 2 copies)
9. ADOPT-215 (original plus 3 copies)
10. ADOPT-230 (original plus 1 copy)
